Please print and then fill out this form as per the sample above (Item #0). Be sure to write down the correct image number from the website. If the image you want is available colorized, be sure to note that in the item description (by putting a c after the number, e.g. vw104c). If you are interested in custom colorization for a black and white image, please contact us to get a price quote. Remember, you can print any of the b/w images in color on your inkjet printer. If you do not include a "Name of your Custom CD" we will use your name.

Please Note: We are sorry, but Custom CDs cannot be ordered electronically at this time. You will have to print out this form, fill it out and email it with your credit card info or mail it in with your check (made out to V V Graphics, 3719 S. 194th Street, SeaTac, WA 98188). Custom CDs can include up to 15 images of your choice and are $29.95 each. The shipping and handling charge is $7.50. If you live in Washington State, you will also need to pay sales tax of 9.8%.
